Health Insurance Marketplace Fee Exemptions

If you don’t already have health coverage, the Health Insurance Marketplace offers a way to find and buy health coverage. Starting in 2014, every person in the U.S. must have minimum health coverage or must pay a fee on their federal tax return.

Members of federally recognized American Indian tribes or those eligible for services through an Indian Health Services provider may be eligible for an exemption from the fee.

Use this application if you or anyone in your tax household is a member of an Indian tribe or another individual who’s eligible for health services through the Indian Health Service, tribes and tribal organizations, or urban Indian organizations. Complete the application and send it to the address on page 3 of the application.
